Adult Services

Jubilee House provides a range of residential services within Welwyn Garden City for adults over the age of 18. To access the services people must have a learning disability, but people who have associated physical disability and other disabilities are also able to use our services.
Our adult services include The Meadows Short Breaks Centre, three registered care homes and our new Supported Living scheme. These are commissioned by Hertfordshire County Council and this means that normally people wanting a service would speak with their social worker and a referral would come to us through their social work team.

As with all our services, it’s essential that the adult's services deliver the support and care that people want and need. To achieve this, any prospective user is fully involved in the assessment, transition and induction procedures together with their families/friends and others important to the individual.
Opinions, choices and preferences are sought to ensure we build with them, a person centred package of support.
Many people are not able to communicate in conventional ways, which does present challenges, however with the involvement of people who know and love the individual we are able to work together to identify their needs and wishes and provide support according to these.
To assist in the delivery of appropriate and relevant services, every user has their own support plan which details levels of independence, personal goals and support needed to achieve these. Each person has their own key worker who works with the service user in monitoring and reviewing their support package and helps to coordinate the involvement of all people important to the service user.
Jubilee House is committed to providing high quality services and in order to ensure we are delivering services people want and need, we operate various quality monitoring systems and procedures which include receiving feedback from the people using the services.
